I understand that I will be participating in controlled physical contact with other camp participants during the on-ice sessions and off-ice activities.

We the applicant and her/his/their parents or guardians agree that Sommer Hockey, Mike Sommer, Keltie Sommer, Keltie Sommer Counselling & Performance Consulting, and instructors, along with the recreational facilities and their employees are not liable for any incident, injury or loss however caused by participating in the Sommer Hockey Camp and agree to release the same from claims and damages.

We further agree that the participant has no medical conditions and that we will not be held responsible for all medical and dental claims and/or insurance.

We also agree to adhere to the decisions regarding player grouping, training and format put forward by the Sommer Hockey Camp.

Participants agree that they will not attend any Sommer Hockey Camp session if they are experiencing any symptoms of illness.

Communication
Should we need to contact you, we will reach you via the telephone number and/or email address you have provided. You may contact our camp via email at connect@sommerhockey.com and we will generally respond to emails within two business days. (Please be aware that email is not completely confidential.)

Payment & Cancellations
Payment for the camp (deposit or full) is due at the time of registration via e-transfer.

If you need to cancel your participation in the camp, please note that the deposit payment will be non-refundable after February 1, 2026. After June 1, 2026, the full camp payment will also be non-refundable.

If you are attending more than one camp this Summer 2026 and/or if a second family member is attending a camp, you will receive a discount of $50 for the second registration.

Privacy & Sharing of Information
I authorize Sommer Hockey to collect my personal information and I also understand that my personal information is confidential and will only be disclosed to third parties with my permission.
